Care Management Services: Service

HDSW is a New York State Designated Provider of Adult Behavioral Health Medicaid Home and Community Based Services (HCBS).

HDSW has been approved to provide the following HCBS Services:

* Psychosocial Rehabilitation (PSR)

* Community Psychiatric Support and Treatment (CPST)

* Habilitation

* Family Support and Training

* Education Support Services

* Empowerment Services-PeerSupports

* Prevocational Services

* Transitional Employment

* Intensive Supported Employment (ISE)

* Ongoing Supported Employment

* Short-Term Crisis Respite (Coming Soon!)
