HDSW has been involved in the evolution of community based direct-care services for vulnerable populations in NYS for almost fifty years. In today’s ever-changing healthcare marketplace, HDSW provides person-centered services to over 3,000 individuals and families each year. HDSW has developed key business partnerships in order to achieve the Triple Aim of improving an individual’s experience of care, improving their health and reducing the cost of providing care.
HDSW is a founding/owner member of Coordinated Behavioral Health Services, Inc. CBHS is a 501 © (3) organization, comprised of progressive, community behavioral health and disability service providers serving 7 Counties in the Hudson River region.
CBHS maximizes the intellectual capacity and resources of its members in order to meet the challenges of a rapidly changing healthcare system, to successfully integrate and manage care and provide the cost-effective outcome- based services required for sustainability.

Montefiore, renowned for its long-standing commitment to community-based healthcare, is leading a group of nearly 250 healthcare providers, community-based organizations, local government officials and more, from across Westchester, Rockland, Orange, Sullivan, Dutchess, Ulster and Putnam counties, to form the Montefiore Hudson Valley Collaborative.
